Upper or Lower Eyelid Surgery? The Guide That Starts With Your Brow

Before we talk about eyelids, look higher. A surprising number of "heavy upper lids" are not a lid problem at all. They are a brow problem: the brow has descended with age, pushing skin down over the eyes. Remove lid skin without recognizing that, and you take too much skin and tether the brow even lower.

Why the brow gets checked first

Brow position must be assessed before any upper blepharoplasty. When the brow sits low, the correct operation is a brow lift, not more skin removal. Sometimes the answer is both, but the brow leads.

This is one of the most common misdiagnoses in facial aesthetics, and it is exactly why a careful consultation measures brow position, lid skin, and the relationship between them instead of just scheduling a blepharoplasty. The hallmark of a good surgeon is knowing when the right answer is a different procedure.

The modern brow lift is usually endoscopic: 3 to 5 small incisions hidden behind the hairline, with a camera guiding the work. It is far less invasive than the old ear-to-ear approach. Recovery runs 10 to 14 days, with some temporary scalp numbness or itching for a few weeks, and the result typically lasts around 10 to 12 years.

If your heavy lids come from a low brow, a brow lift opens the eyes without touching the lids at all. If both the brow and the lids have aged, the plan addresses both, in the right order.

Older approaches still exist for context: the coronal lift runs ear to ear and is considerably more invasive, while the direct lift places incisions above the brow and leaves visible scars, so it is rarely used now.

Who is a candidate, and what is the day itself like?

Candidates are healthy adults, typically 35 and up, with excess upper-lid skin or puffiness, or under-eye bags that never leave no matter how much you sleep. Expectations should stay focused on the lids: eyelid surgery refreshes the eyes, it does not redesign the whole face.

The surgery itself is an outpatient day case, done under local anesthesia with sedation or general anesthesia, and takes 1 to 2 hours. Stitches come out at 5 to 7 days. Cold compresses and head elevation carry the first few days. Eye makeup waits about 2 weeks, contacts about 2 weeks, full exercise 4 to 6 weeks. The final result lands at 2 to 3 months, with the upper-lid scar fading to near-invisibility inside the crease.

Upper and lower lids: what each fixes

When the problem genuinely is the lid, upper blepharoplasty removes excess skin, and sometimes a little fat, through an incision hidden in the natural lid crease. The procedure takes 45 to 60 minutes. Most people are back at work in 5 to 7 days.

The change is subtle but specific: hooding gone, the crease visible again, eyes looking rested instead of tired. When hooding is severe enough to block the upper field of vision, the surgery can improve sight as well as appearance.

Lower blepharoplasty addresses under-eye bags, prolapsed fat, and crepey skin. The surgeon works below the lash line, or through an incision inside the lid itself, the transconjunctival approach, which leaves no external scar at all. Fat is repositioned into hollows or selectively removed, and excess skin is trimmed.

It takes 60 to 90 minutes, and bruising is more noticeable than with upper lids. Plan 7 to 10 days before work. Upper and lower are frequently done together, the four-lid blepharoplasty, in a single session.

Risks, honest numbers, and how long it lasts

Two findings that look surgical but are not. Dark under-eye pigmentation is a skin-color issue; no scalpel fixes it. And certain eye conditions, dry eye, glaucoma, thyroid eye disease, must be evaluated before anyone operates near your eyes. A surgeon who skips that screening is a surgeon to skip.

The common effects are temporary: bruising, swelling, dry or watery eyes, light sensitivity, blurred vision, mild asymmetry. They resolve.

The rare ones are why surgeon experience matters. Retrobulbar hemorrhage, bleeding behind the eye, is rare, and permanent vision loss rarer still. Both stay small in experienced hands at accredited facilities with proper monitoring. Lower lids carry their own specific risk: the lid can sit slightly differently afterward or, uncommonly, not close fully. Conservative skin removal and sound technique keep that risk low.

Upper eyelid surgery lasts 5 to 7 years by most accounts, with some sources stretching that to 10 or more. Lower eyelid surgery is often a once-and-done operation. Aging continues everywhere else, which is why the rest of the face eventually catches up.

Some context on how common this has become: eyelid surgery is now the number one surgical cosmetic procedure in the world. The 2025 ISAPS global survey [1] counted more than 2.4 million procedures, the first time it topped the list for both women and men.

Frequently asked questions

Q: How do I know if I need a brow lift instead of eyelid surgery?

A: If the brow has descended with age, it pushes skin down over the eyes and mimics heavy lids. Brow position must be assessed before any upper blepharoplasty, because removing lid skin without recognizing brow descent takes too much skin and tethers the brow lower.

Q: How long is the recovery?

A: An endoscopic brow lift runs 10 to 14 days. Upper eyelid surgery: most people are back at work in 5 to 7 days. Lower eyelid surgery: plan 7 to 10 days, with more noticeable bruising.

Q: What are the serious risks, in numbers?

A: Retrobulbar hemorrhage (bleeding behind the eye) is rare, and permanent vision loss rarer still. Both stay small in experienced hands at accredited facilities.

Q: How long do the results last?

A: Upper eyelid surgery lasts 5 to 7 years by most accounts, with some sources stretching that to 10 or more. Lower eyelid surgery is often a once-and-done operation.

Q: How common is eyelid surgery worldwide?

A: Very. The 2025 ISAPS global survey counted more than 2.4 million eyelid procedures, making it the number one surgical cosmetic procedure in the world for both women and men. [1]
